--is male.
--is a hero, not a villain.
--wears a costume.
--is non-super-powered.
--has been a part of a team, though not the Justice League, Justice Society, All-Star Squadron, Infinity Inc., Freedom Fighters, or Seven Soldiers of Victory.
--has been a founding member of a team.
--debuted prior to Crisis.
--is from Earth-2 (though it's ambiguous) rather than Earth-1.
--Adventures in the 20th century
--has never been published by Fawcett.

It is Percy Sheldrake, the original Squire (later the original Knight).
He first appeared as the Knight in Batman #62 in 1950 (which I guess counts as Earth-2?), before joining the Batmen of All Nations in 1955.
Young All-Stars retconned him into having served as the first Squire to the Shining Knight, and as a member of the Young Allies.
And he was later succeeded as the Knight by his own son and Squire.

Recap:
- a hero in the beginning
