None of the above.
CLUE: This is a very well known DC villain.

I’m trying to think of villains with smoke or fire...
Two-Face? I seem to remember him always with a cigarette

Correct! Kool Cigarettes had a penguin mascot named Willie who wore a top hat, bow tie and monocle and smoked a long cigarette (of course) and Kane and Finger used him as inspiration for their Batman villain. Wow. I thought you meant in story inspired by. Like Captain Boomerang was the mascot of a toy company.
Why was Earth-B designated Earth-B?

Because it consisted of stories published in "The Brave and the Bold"?

Piggy-backing on CMK's guess:
It was for some of the stories Bob Haney wrote?

Yup. Earth-B was for all those stories that did not fit into Earth-1 continuity. Usually written by Bob Haney. sometimes for Brave & Bold, but not all Brave & Bold stories took place on Earth-B

Since I wouldn’t have gotten that without the inspiration from CMK, and I just asked a question, I’ll let him have the next question if he wants.

Sure, I'll give it a shot.
Whom did Renee Montoya resurrect with the Spear of Destiny?

Crispus Allen? (I think that was the name of the Gotham Central character.)

Neither of them. Crispus Allen is indeed an important character in all this, but he was not returned to life.
