Well, there was Cos after the hussy took advantage of him and mind wiped him.
Then there was Superboy and Supergirl that were given amnesia whenever they went back to the past. That's two more.
There has to be more so let's stick with this one.

Identity Crisis, in which it was revealed that some of the JLA had mindwiped Batman of their mindwiping of supervillains.
In Adventure Comics 350-351, the Legionnaires take the amnesia of Superboy and Supergirl a step further by making them forget everything they know about the Legion and the 30th century.
In Red Winter, Doctor Light (Kimiyo Hoshi) and Flash (Wally West) are given amnesia and brainwashed by Sonar into becoming his slaves. He later does the same to several more heroes: Rocket Red Dmitri Pushkin, Black Canary, Metamorpho, Huntress, Tasmanian Devil, General Glory, Mister Miracle, Alan Scott, Jay Garrick, Wildcat, Al Pratt and Doctor Mid-Nite.
