Lightning Lad lost an arm, Vi lost a leg, & Triplicate Girl lost a body. But those were more in the line of duty than an acident.
Wildfire lost his whole body (but he got better) in an accident.
And didn't Dawnstar get her wings cut off?
That's all I can think of.

It was in Who's Who in the Legion # 7, that much I remember clearly. I haven't had the chance to dig up that issue from my collection in the past two weeks, but maybe I'll get there soon.
