Morgan was killed off by none other than Mike Grell during the current Warlord series (which is ending in an issue or two). I thought Catalina met her end in the Secret Six arc in which she appeared, which was in the first few issues of the title.

Part of the problem is that the ease of "bringing back" characters makes killing them off that much easier. And that much more pointless, except for the gratuitous shock-value of it.
