Does it have something to do with having their internal organs exposed for some length of time? Or perhaps with walking around with no clothes on?

That's even closer, but lots of characters have done that without qualifying for this list. The key point is not so much what the characters are wearing as how the characters are depicted. I will also point out that only male characters are on this list.

That's just as close, but lots of comic book characters have walked around naked in public without qualifying for this list. But there's something about how Dr. Manhattan, the Dark Destroyer, Professor Hyatt and, to the best of my knowledge, Peter Parker, have been depicted on one specific occasion each that sets them apart from all other mainstream comics characters I've heard of. And you're all coming real close to saying it, but nobody's said the one key word or idea that convinces me you've got the answer.

... I guess that's close enough. (And probably true.) The key thing is that all three characters (Dr. Manhattan, obviously; the Dark Destroyer in a panel where he's a newborn baby; Prof. Hyatt in cross-section on the cover of a recent issue of The All-New Atom) have had their genitalia depicted in plain view in a DC comic.
ETA: And I should have said, go ahead.
